Waiver of Liability (WOL)
A form received for non-contracted provider payment appeals; its receipt date goes in Table 3, Column J.
Table 3 Column J records the date the WOL form was received for non-contracted provider payment appeals. Enter None for ODs, enrollee-submitted requests, or if a WOL was never received. A copy of the WOL is part of the supporting documentation for NCP payment reconsiderations.
