Upgrade to expedited
When a request first made as standard is later asked to be expedited, by the enrollee, representative, prescriber or the sponsor itself.
Tables 1, 2 and 4 ask whether the original request was standard and later requested to be expedited, and record the date and time of that upgrade request. The upgrade fields take None if the initial request was expedited, the sponsor chose not to expedite, or the request was received and processed as standard.
