Tracer case summary
A written storyboard or dashboard showing, in time order, how the organization investigated and resolved a selected compliance issue.
CMS requests 6 tracer case summaries. It picks cases that represent compliance risk and are likely to touch several compliance program elements, such as pharmacy benefit management, appeals and grievances, or an FTE performing a delegated function. Each summary must cover the issue, the units involved, root cause analysis, actions taken, corrections with a timeline, escalation, communications, and prevention controls, in chronological order.
