Prescriber supporting statement
The physician's or other prescriber's statement backing an exception request. Its receipt starts the exception decision clock.
For exception requests, the sponsor must notify its decision within 72 hours (standard) or 24 hours (expedited) after receiving the supporting statement. The layouts record the date (and in Table 2 the time) it was received; if it came with the initial request, enter the date the exception request was received. Enter None if none was received.
