Prescriber
The prescribing physician or other prescriber. Coded P when they made the coverage request.
In CDAG layouts, P in the 'Who made the request?' field means the prescribing physician or other prescriber made the request. If a prescriber requested coverage, auditors check that the decision notice was also sent to the prescriber. Exception requests depend on the prescriber's supporting statement.
