Interdisciplinary Care Team (ICT)
The team managing an enrollee's care, made up of clinical disciplines set by the plan's approved Model of Care, plus specialists when needed.
CMS checks that each sampled enrollee's care was managed by an ICT of appropriate clinical disciplines per the SNP's approved MOC, including specialists when needed. The ICT is specific to each enrollee's individualized needs. CMS also reviews whether the ICT helped create and update the ICP, held reviews at least annually, involved or invited the PCP, and received MOC training.
