Individualized Care Plan (ICP)
An enrollee's care plan, built to address needs found in the HRA and consistent with the plan's Model of Care, with measurable outcomes.
CMS checks that an ICP was completed for each sampled enrollee, designed to address HRA-identified needs and consistent with the MOC. It should include measurable outcomes with a timeframe, the enrollee's self-management goals, personal healthcare preferences, services tailored to the enrollee's needs and whether goals were met. ICPs must be reviewed or modified as health care needs change and must actually be implemented.
