Health Risk Assessment (HRA)

An assessment of an SNP enrollee's needs (medical, psychosocial, cognitive, functional, mental health) that the plan must complete on time.

CMS tests whether the HRA included a comprehensive initial assessment and reassessment(s) of the enrollee's needs, for example medical, psychosocial, cognitive, functional and mental health needs. The ICP must be designed to address needs identified in the HRA. HRAs come as an initial HRA (IHRA) and annual re-assessments (AHRAs).
