Enrollee's representative
Someone acting for the enrollee, including a purported representative. Coded ER in the 'Who made the request?' field.
ER marks a request made by the enrollee's representative or purported representative. When a representative is involved, the sponsor records when it received the Appointment of Representative (AOR) form or equivalent written notice. Auditors check whether notice also went to any representative the enrollee identified.
