Dismissed request
A request the plan closed without a coverage decision; entered as Dismissed with the dismissal date.
Dismissed is one of the three request determinations. For dismissed requests the Date of Determination is the date the plan dismissed it, and the issue description must give the reason for dismissal. CMS samples 10 dismissed requests to check whether they should instead have been treated as a coverage request or grievance.
