Deficiency
A finding or issue identified during a compliance oversight activity. The COA universe records how many there were and describes them.
COA Column I counts the deficiencies, findings, or issues an activity identified, and Column J summarizes them. Column K records whether any of them required a corrective action. Tracer case summaries must explain the steps taken to correct deficiencies.
