Appointment of Representative (AOR)
The form or equivalent written notice by which an enrollee appoints a representative; its receipt date is recorded.
The layouts record the date (and for expedited cases the time) the AOR form or equivalent written notice was received by the sponsoring organization. Enter None if no AOR or equivalent notice was received or required. For requests from a representative, a copy of the AOR is part of the supporting documentation.
